I2PL - The Next Generation of IPL
Elizabeth VanderVeer, MD Portland, OR

For the last several years, Intense Pulsed Light (IPL) technology has become the gold standard in skin rejuvenation as people have shifted away from invasive procedures with prolonged recovery periods, toward non-ablative methods with no downtime. The basic science behind IPL is based upon the principle of using the spectrum of visible light to target colors in the skin. The most commonly undesirable skin colors are red (diffuse redness, dilated capillaries, Rosacea.) and brown (age spots, sun spots, melasma, and more). In addition to targeting these two colors—red (hemoglobin) and brown (melanin). Some IPL machines are capable of skin rejuvenation in the form of pore size reduction, acne scarring improvement, and softening of fine lines and wrinkles. These results are achieved by collagen remodeling as a result of heat delivered to the skin by the IPL device. IPL devices emit a broad spectrum of visible light in a variable level of energy wavelength. Second generation machines, known as I2PL, have eliminated wavelengths not required for the task of photo skin rejuvenation, and are therefore more effective at targeting all aspects of skin rejuvenation. This is accomplished by using carefully selected wavelengths of visible light delivered to the skin in short pulses with minimal discomfort.
The results can be immediate, but full rejuvenation (including collagen synthesis) takes weeks to months and a series of treatments (usually four) are required to achieve optimal results. Beginning treatments are spaced a month apart, and maximum results are seen 90 days after each I2PL session. At the end of the treatment series, patients are encouraged to follow a maintenance plan based upon their lifestyle (sun exposure, etc) and original skin condition (rosacea vs. limited sun damage). Maintenance typically requires three to four single visits per year. As with most light-based systems, skin color is extremely relevant. The source and nature of the pigment makes no difference to an I2PL. Any enhancement of melanin, whether by sun, self-tanners or naturally darker skin, will compete with the intended target of the I2PL. Since this will result in a burn to the skin rather than the desired outcome of the treatment, patients are advised to avoid all “sun” before, during, and after their I2PL treatments in order to obtain optimal results. This is such a critical component of the success of I2PL treatment that a reputable physician will turn patients away for treatment if their skin is too dark.
Ideally, I2PL therapy takes place where there is a well-trained physician on site, an experienced technician and the highest-caliber equipment available. The prospective patient is encouraged to visit a medical center (vs. a Medi-Spa or tanning salon) to consult with the staff. It is also reasonable to have a test spot performed if either patient or medical provider has any concerns about skin type or condition. Potential patients should also ask to see pictures of the medical center's own patient results. This point is vital, as far too many practitioners rely on device manufacturer pictures that can be easily downloaded and/or manipulated from the internet. The results of effective I2PL treatment can be phenomenal. At VanderVeer Center, we have two second generation IPL machines (by Danish Dermatologic Development - The Ellipse and The Flex), and have had countless patients with fantastic outcomes. Our program consists of careful, expert analysis of skin type and condition(s) to be treated; patient goals; and our assessment of what we believe are realistic expectations for patients to have as they embark on a rejuvenation program. Treatments are in-office procedures requiring no anesthesia. Patients have no downtime and can return to normal activities immediately after each procedure. One caution for our male patients includes the distinct possibility that any dark hair on their face and neck will possibly be permanently reduced or removed. Most men find this an added benefit to their treatment, but they must be forewarned. When a high-quality I2PL machine is applied to the appropriate patient, non-surgical skin rejuvenation is achieved in a safe and effective manner, resulting in outstanding results and a very happy patient and provider. Now that really is a gold standard.
